Item name - Tyball's Flask Stand (Artifact Rarity 9)
Location - Underworld
Nature of bug: The item can be seen from an adjacent room by use of the ctrl+shift feature, but the teleporter that would normally take you into this room doesn't seem to exist here on Evo. I referenced a couple of websites regarding this item and attempted to reach it with no success. Even with the help of Jessianna, we were unable to gain entrance to that area.
Suggestion for easy fix(s) - Install a teleporter to function in a manner consistent with the current guides, or relocate the stealable to a new location somewhere else in the underworld that is accessible.

i've noticed that one myself aswell... i did however never try to go for it. Did you guys try the following? :

Successfully passing the Slimes and Acid Slugs, you will see the door of the Puzzle Room. Continuing down the stairs to the left, you will come to a room with magic vines on the walls: You notice something odd about the vines covering the wall. If you use Acid Sacs (which can be obtained from the corpses of Acid Slugs) to these walls covered with vines, you will reveal a dark passage to a hidden chamber. Tyball's Flask Stand can be found here but the main purpose of this room is get one of the keys needed for the quest to access the Stygian Abyss Dungeon.

There is no access to the puzzle room. Where the magic vine should be, the wall was removed. And it is a non entrance void.  Tried several different ways to see if there was a tele there. But was not able to find it. 
As you can see the same information you loaded is the quest to get into the abyss. This whole starting quest line is not active. Instead we get to go straight into the abyss (which I do not mind hehe).

The only way to reach those places was done by using the restorative soil before it was nerfed so now, all those place are unaccesable unless you marked runes prior to the restorative soil change! (Which i did)

The area in question is there. The stealable can be seen by using Ctrl+Shift from the room directly north of it, if you are standing at the southern edge of the room. Since Razor's Positioning system doesn't recognize the Underworld, I am unable to provide you with precise coordinates. If you like, I'd be more than happy to show you exactly where it is.
